The trick is that less is more, I've found.
Also, follow the tutorials as they help a lot. I dunno if what I do is correct, but at about 11k metres, I start to turn east 45 degrees and do that til the apoapsis is maybe 150-200k metres, then cut engines. Get within 1 minute to the apoapsis then boost towards it (the yellow circle) until the periapsis appears and just level it out.
Here's the ship I got to the moon with, and also about 80billion metres (but with no return):
In the middle is a separator with another engine behind it.

Are your RCS ports not located directly on the pitch/yaw axes? If you offset a quad symmetry so that they're 45 degrees off like that, I know your situation will happen.bennisboy wrote:
