Like many of you, I hold a certain level of respect for most people, but to get beyond that it must be earned.

The above quote is how I live in relation to authority. That said, I believe there is a difference between being respectful and respect. Being respectful seems to be an outward acknowledgment of authority (sir, ma'am, officer, etc) when interacting with authority. That is what is expected of a mature person. However, I do not internally have to like them or agree with them. There were teachers and police officers that I have encountered in my life that I didn't like at all. But what would disrespecting them prove? That I can be a prick to someone I don't like? That would only lower me to their level and likely make my life unnecessarily miserable.
I feel respect is lost more than it is earned. You will automatically have respect from me until you give me a reason not to. You may also earn a greater level of respect from me depending on the consistency between your words and actions. Therefore, I think respect is lost more than it is earned. If I go about life demanding people not giving anyone any slack until they earn my respect I'll probably be known as a real jerk.
