Not photoshopped, and there was no lag whatsoever on the server. Everyone's ping showed as 999 no matter what.

I think it is a server side thing and have been on some like it myself. The clan who ran it said that it stopped people disputing pings etc, and flaming people to get kicked because of a 3 digit ping.
