A shot to the head generally mashes up your brain so that even if you do survive, you'll either be in shock or have had so much of your brain displaced, destroyed or damaged that you will not know what the fuck is going on if you somehow manage to stay conscious. There are cases of people being shot in the head 6 times and surviving and another of a boy shot with a .44 Magnum point blank losing 1/3 of his skull and surviving. A shot to the head is no guarantee of death let alone a painless one.
Decapitation severs the brain from the central nervous system and kills you instantaneously, I know of no survival cases of people being decapitated but if you find any I'm willing to listen. But until then Decapitation is the only sure way to kill someone and potentially much less painful.

Doubt it, since I'm sure you're aware most contractors and soldiers in the 2 conflicts would rather get shot fighting than be captured and beheaded.
