No you don't pay a monthly fee. It's kinda multiplayer - it's really singleplayer but the creatures on your world are real creatures that people from around the world have created, and the game servers or whatever just put them in your personal world.

So lets get this strait, I cant play this game online with my friends and have are Penis's band together to take over a planet or something like that? so its single player but other people creatures are created in my single player game based on what other people build? Can my creature Mate with other Creatures( so a penis monster and and vagzilla can mate?
