I need a new video card, my 9800GT keeps freezing. It rarely freezes in call of duty or quake III, but freezes every few minutes in Heroes of Newerth, All Points Bulletin, and other more intense games. I'm not looking to upgrade too much, just replace. So far I've narrowed it down to these three choices:

MSI 250 GTS 1GB
PNY GTS 250 1GB

XFX Radeon HD 4770

Anyone have any recommendations or opinions on these cards? My budget is ~$130 USD. My case and room get pretty warm which could be why my card failed, so my new card should be able to handle the temps.

E6750 @ 3.2GHZ
Gigabyte P35-DS3L
2GB DDR2-800
575w Antec PSU with 25A on one of the 12V rails

The HD 4870 would be a good choice but if you can save a little more then go for the  HD5770 instead.

5770 would be a great choice, I'm really happy with mine. Would only cost you upwards of an extra $20, or $30 if you go with the XFX card (my recommendation)

I was thinking just before I ordered, how can I be certain it's my card that's failing and not my power supply not giving it enough juice?

You easily could run a 5770 on a good 500W PSU (manufacturer's recommendation for crossfire is a 600W PSU, but I'd go higher tbh), probably even a 450W PSU without hassle, so I think you're fine there.

Well what if my power supply is failing or broken or something and it's to blame for the freezing instead of my video card?

Well then, you're going to either just buy the card and hope it's not the PSU, take it to a shop and get your computer checked out (will cost a bit), buy a new PSU and hope it's that and not the card, buy both card and PSU, or probably my best advice is to wait for someone who knows a lot more about this than I to answer your question.

To me, if it's freezing during more graphically intense games, it sounds like the card. But I can't say for certain.

But if you're in a hurry, they're all that I can see you can try.

My dad's a professional electrical engineer, I'm sure he has some meter that can read the output of a powersupply. I think I'm gonna try this and see if everything shows up.

If all else fails just remove anything that isn't vital for your computer to run, so any extra hard-drives, dvd drives, etc etc, and run the game. If it stops freezing then it might well be the PSU.

You should try reinstalling the drivers with Driver Cleaner...
