The blood effect looks like someone has thrown jam on the screen from inside the TV. And I'm pretty certain, in a couple of those shots, he shot the guy, injured him, then killed him and still got the "1 Shot Kill" bollocks.
Even if hardcore mode removes all that shit from the screen, the only game modes that you can play hard core are:
