What exactly do you need to do to get the 2nd tier unlocks?

You need to play SF and unlock the weapon there and take it back to vanilla BF2 BUT in order to unlock them in SF do you start in the ranks as Pte and work your way up or do I carry my BF2 rank and need to work to the next rank to get an unlock in the SF world?

Obviously I haven't played SF yet but I ordered it some days ago and I should start shortly.

What is the score in EF? Do you carry the BF2 rank or start again?

The ranks are the same, but you will get additional unlocks if you buy SF ( provided you have enough global points, of course ). For example, I got 4 additional unlocks when I logged into SF for the first time. But as you use the same account for vanilla BF2 and SF, those additional unlocks ( earned from SF ) can also be used to unlock weapons that are still locked in BF2.

the rule is that if you want to unlock a SF weapon for use in vanilla BF2, you need to have the first unlock for that kit unlocked already in BF2. For example, if you play Spec Ops and don't have the G36C unlocked in BF2, you cannot unlock the SCAR-L from SF.

EF is just a booster for vanilla BF2. You play with the same account as in vanilla BF2 and SF.

I've got all the first tier unlocks except the Assault.  So I should expect to be able to unlock all of those as soon as I install SF and start playing and then take em back to vanilla BF2?

hmm, I have SF installed, played it last night.  Just got promoted to gunnery sargeant today, and didn't get any additional unlocks (except for the standard one).  Any idea why?
