DICE chose one.
That's all one can say about it. The first one is said to have been chosen because he had had 2142 unique dogtags or something. I don't know about the second one and so far, there hasn't been a third one.
So, obviously, this rank is manually awarded by whom they consider to be worthy. No one (and i mean NO ONE) knows for sure how they do it, most likely not even they do until they have agreed on one.
