Or use Royale Noir like I am which needs nothing else but the theme file to work.

Surprise, surprise. Windows does have skinning built in. As I mentioned, use the uxtheme.dll patch by Radish, which enables skinning for non-official styles in Windows XP without a third-party application running constantly.

His didn't have the stacklet included, which is what makes the toolbar genuinely useful (for me anyway).

Icons - are you looking at the windows ones? They have been packaged in a zip for me?
See the icon in the upper left hand corner. It says it's a "zip" file, but I can't unzip it and I don't know how to use it as a theme. It's from this page
