I think it's most kills then fewest deaths then teamwork points. but i'm not absolutely sure. I've won and lost medals based on deaths. a tie would probably require identical kills, deaths and teamwork points.
it goes by who has the most kills.if both players have the same number of kills then it goes to the player with the least amount of deaths.if they have the same amount of deaths...then i have no idea.

I thought that it was decided by rank. For example, two players have the same total score in a round which is better than anyone else in the server, the gold star goes to the Captain instead of the Sergeant.
But this is just what a friend told me, others seem to think that it depends on your kills. They're probably more reliable than my friend.

If both are same rank aswell, i think it goes to who has more points overall. like when 2 people of same rank go for commander.

It's points, kills, deaths as I recall. Though the last time I was tied on points AND kills was a long time ago, so I'm not quite sure.
As for the commander, in case of a tie, the soldier will always get priority.

First it goes by # of Kills
If kills are the same then its Least # of Deaths
If thoes are the same then its Random.
If kills are the same then its Least # of Deaths
