As far as i'm concerned it's all Valve's fault.
I remember a few months after the first game came out, when people were wondering where all our promised content was, and why Valve was doing nothing to counter closet camping and ragequitting. Somebody made a thread on the Steam forums suggesting that Valve realised that the game has such deep rooted problems, that can't be fixed with a few quick fixes, and had abandoned everything and started again to try and make a less frustrating and broken gameplay experience. One that wasn't plagued with ragequitting and the like.
Instead it seems that the L4D team did nothing. Nothing. They knew the problems and didn't try and think of an elegant solution to anything. As usual, the fault lies with them.
