I squeeze the foam, insert it with the shure logo upside-down, twist it 180 degrees and loop the cable around my ear. Seems to be the standard way of doing it...

Edit: In response to the Shure questions- squeeze the foam insert so that it is compressed, loop the earphone over the top of your ear so that the 'Shure' text is the right way up on the earpiece, and then insert deep into the ear canal but not so that is causes discomfort. The natural curve/shape of the earphones will hold them in place, as well as the looping of the cable over and around your earlobe. The foam will expand and within 5 seconds you'll have a snug-fit state of sound isolation .
