Uh............right. It's really not that hard. You print it up like a credit card, you know with the raised numbers? Only the numbers are letters, which spell out name. Every family gets a card with all their names on it. You go to the doctor, and you have your card and ID (e.g. drivers license, passport, etc.). The government organisation responsible (in our case, Medicare) pays, either through the doctor bulk billing, or through re-imbursing the patient (atm the biggest problem is a lot of doctors don't bulk bill, meaning you have to have the cash to go in the first place). Also, there are incentives for those who can afford private healthcare to get it.

We are angry at the oil companies because when we goto the pump we see the cost immediately. Remember that stink a couple of years ago about the rising Bank Fees? The $1 charges banks charged to use ATMs...those type of charges people see, whereas taxes that are taken out of your check every 2 weeks you don't see and thus think its free.

US defence expenditure could easily be cut to pay for a totally free healthcare system, whilst still being at least 50% higher than the next biggest defence spender.
Taxes wouldn't need to go up for US citizens to have free healthcare. You could just cut the defence budget a bit.
