I had a group of guys in a jeep which I was driving, and tried to take a short cut over a "hill" which turned out to be a little steeper than I thought and flipped it, killing all. I took it again the next time solo ok. Only crashed when it mattered..

Stoping a jeep or boat or jetski , getting off only to have it touch you or squeeze you against something & you die.
I was sniping on mastur city as commander, so i decided to have my very own supply crate. So i gave myself one and issued some orders and places some arty. Next thing i know i got crushed by the crate that i droped myself.
