If you're "only" counting IBM compatible PCs: TSeng ET4000 with 256 KB of memory (that was a LOT back then). I still have one of those cards lying arond in case of dire emergency and a need to boot from an ISA slot (not many boards LEFT with ISA though).

ATI rage pro 128. yeah it sucks, but with my p3 1GHz it actually was pretty good for the time
once upon a midnight dreary, while i pron surfed, weak and weary, over many a strange and spurious site of ' hot  xxx galore'. While i clicked my fav'rite bookmark, suddenly there came a warning, and my heart was filled with mourning, mourning for my dear amour, " 'Tis not possible!", i muttered, " give me back my free hardcore!"..... quoth the server, 404.

Matrox Mystique, great for playing quake.

I used to really like the old 3DFX cards though, the Voodoo 3 3000 was great and the Voodoo 5 5500 was an awesome card.
